Investigation Summary
Employee #1 was cleaning the chain and sprocket of an exercise bike. As he knelt down, his weight was placed on the pedal of the bike, which caused it to move and his right hand little finger was taken through the sprocket. This action resulted in the amputation of the finger tip below the first interphalangeal joint. He was not hospitalized.
