Investigation Summary
At approximately 7:15 a.m. on April 6, 2006, Employee #1 was instructed to begin carpentry work on an elevated level. He used a 10 ft stepladder, already positioned, adjacent to a wall near his assigned work area. He climbed up the ladder and placed his coffee cup and saw on the top of the elevated sub floor. His tool belt and bags were on his body as he climbed. He leaned over to get his body up and onto the sub floor and the ladder kicked out from under him. He may have been standing on the step below the top cap of the ladder or the top cap. As he fell, he grabbed onto something to stabilize himself, but was unable to halt his fall to the floor below. He fell approximately 10 ft and fractured his lower arm. He was hospitalized.
