Employee Dislocates Foot When Forklift Tips Over

At approximately 11:40 a.m. on April 22, 2005, Employee #1, a certified forklift driver, was loading recycled bales of cardboard on a flatbed truck with a forklift. Employee #1 turned the forklift and the forks of the forklift were extended up, creating the lift to be off balance. The forklift tipped over dislocating Employee #1's right foot. Employee #1 was taken to San Francisco General Hospital, and he was hospitalized. The surface area of the accident indicated that there were no potholes or unusual surface conditions that would have contributed to the forklift tipping over. There was no evidence to indicate that there was anything broken or wrong with the forklift.
