Investigation Summary
At approximately 10:20 a.m. on December 27, 2004, Employee #1 was working with coworkers, loading cargo pallets onto a string of dollies drawn by a tow tug inside a warehouse. As Employee #1 was attempting to lock the pallet latch on the dolly by hand, the tow tug operator moved slightly. This caused the pallet to jolt against the latch nipping off part of Employee #1's finger. Employee #1 was hospitalized with an amputated finger.
