Investigation Summary

Investigation Nr: 201125101
Event: 09/08/2004
Employee fractures leg while moving pallet

At about 9:30 a.m. on September 8, 2004, Employee #1 was working alone, delivering palettized loads of supplies to customers. As Employee #1 tried to extricate the first load of the day by means of a pallet jack and pulled the load onto the elevated tailgate of the truck, he found it hard to move. Thus, he pulled harder until it suddenly came free. The moving load pushed Employee #1 off the tailgate, and he fell to the ground. He fractured his leg and was hospitalized.
