Employee is killed in fall from scaffold

At 7:03 a.m. on August 25, 2004, Employee #1 was standing on a scaffold 15 feet above the grade. Employee #1 was installing window blinds for his church. During the installation, Employee #1 fell off the scaffold and landed on the concrete floor. Employee #1 was seriously injured and died later in the hospital.
