Investigation Summary
On May 24, 2004, Employee #1 was working with other men upgrading a warehouse building into which the employer was to relocate. Employee #1 was installing lampshades on the ceiling, working from an elevated platform resting on the forks of a forklift truck. The platform suddenly tipped, and Employee #1 fell to the concrete floor of the building and was fatally injured on impact.
