Investigation Summary
At approximately 10:30 a.m. on July 16, 2001, Employee #1 was working on the roof of a new warehouse with a crew of men cutting openings in the roof for skylights. Employee #1 stepped onto an opening that was partially cut. He fell 17 feet through the opening, fracturing his hips. Employee #1 was hospitalized.
