Investigation Summary
On April 2, 2001, Employee #1 was working in an elevator pit on the installation of new equipment. The elevator just above him was suddenly moved by the man working there, startling Employee #1 in the pit. Employee #1 was injured as he tried to exit the pit, when he slipped and fell off the exit ladder. Employee #1's injuries required hospitalization and extensive treatment.
