Investigation Summary
At approximately 7:30 a.m. on March 14, 2006, an employee of a masonry construction company was unloading a compactor from a flatbed trailer. The employee unchained and started the compactor on the flatbed trailer, and drove it toward the rear loading ramps. When the steel rollers of the compactor came in contact with the steel section of the trailer bed, traction was lost between the rollers and the trailer bed. The compactor slid out of control, and the employee jumped from the compactor. When the employee landed on the ground, the compactor immediately fell off the rear of the trailer and pinned the employee to the ground beneath the compactor's roll over protective (ROP) bar. A coworker removed the compactor from atop the injured employee with a forklift. The employee was unconscious and suffered fractured ribs, internal injuries, and brain damage. Paramedics transported the employee to Simi Valley Hospital, where he was admitted and treated for multiple traumas of the face, neck, chest, and abdomen. The employee remained in a coma for two months. An accident investigation revealed that the employee had not been properly trained on the hazards associated with the operation of the compactor.
