Investigation Summary
On February 16, 2006, Employee #1 was driving a Hedger Machine Tractor, Massey Ferguson Manufacturer Number 290, and Serial Number 725912. Employee #1 requested a coworker, a mechanic, to help him fix the tractor, which had broken down. The coworker fixed the tractor and told Employee #1 to oil the cutting shear blades. Employee #1 put oil on left shear blades and then ran out of oil and was going to get more oil to put on the right shear blades. When Employee #1 climbed onto the hood of the tractor, he slipped on some oil and lost his balance. Employee #1's foot was entangled in the hoses, and his leg engaged and activated the hydraulic controls (sickle bar) of cutting shear blades. Employee #1 placed his right hand on the cutting shear blade to support his fall and his right middle finger and ring finger were cut. Employee #1 was transported to the hospital and treated for an amputated at first knuckle on the middle finger and laceration of the skin on the ring finger.
