Investigation Summary

Investigation Nr: 202462867
Event: 02/10/2006
Employee Fractures Leg When Caught Between Van and Lift

At approximately 9:45 a.m. on February 10, 2006, Employee #1 and a coworker were transporting food by van truck to Alpine Elementary School Cafeteria. Employee #1 was standing on the ground in an opening between the van truck and a load gate lift. When Employee #1 reached into the van truck to get a pan of food to hand over to a coworker, the load gate lift malfunctioned, moved upward, and pinned Employee #1's legs. A second coworker quickly went to the outside controls to lower the load gate lift, and Employee #1 was able to get her legs out from between the van truck and the load gate lift. Employee #1 was helped to the ground, while waiting for paramedics to arrive. Employee #1 was taken to Antelope Valley Hospital, where she was diagnosed with a right fibular fracture, and remained in the hospital for 4 days.
