Investigation Summary
On the morning of October 27, 2005, Employee #1, while cleaning the packaging machine to remove cookie residue to prepare for another type of cookie, came in contact with a moving chain and sprocket that was on the conveyor portion of the machine, amputating her finger. She failed to deenergize the machine while cleaning it. Employee #1 was taken to All Care Medical Group, where she was treated and released.
