Investigation Summary
On December 3, 2005, Employee #1 was driving a wheel tractor. Employee #1 was working alone transporting and planting trees when the tractor rolled backwards and crushed him. At approximately 11:40 a.m. Employee #1 was found pinned between the tractor and trailer. Employee #1 was killed.
