Investigation Summary
At approximately 2:45 p.m. on October 17, 2005, Employee #1, with TB Penick & Sons (a structural concrete contractor) climbed into an industrial truck (Bobcat). While he was positioning himself in the seat, he inadvertently hit the bucket control toggle switch. The bucket came down and crushed his left foot, which was on the front edge of the Bobcat cabin. Employee #1 was hospitalized at Kern Medical Center for fractures to his left ankle and toes.
