Investigation Summary
At approximately 5:00 p.m. on August 2, 2005, Employee #1 and a coworker finished work and the coworker drove them back to the yard. The coworker started driving the truck through the front gate which was opened by Employee #1. Employee #1 jumped on the back bumper of the truck to get to his parked car in the yard. The coworker did not stop the truck and Employee #1 fell of the bumper of the truck. He was hospitalized with a fractured left leg.
