Investigation Summary
At approximately 12:52 p.m. on August 20, 2005, Employee #1 of Mission Sheet Metal was climbing on an unsecured 24-ft aluminum extension ladder on a construction site when the ladder was displaced and he fell to the ground. Employee #1 was transported to Simi Valley Hospital where he was hospitalized for treatment of fractured ribs, fractured shoulder bones, and a pulmonary contusion.
