Investigation Summary
At approximately 9:30 a.m. on March 21, 2005, Employee #1, with Kennco Plumbing, was installing new home tract plumbing. He was engulfed by a dirt collapse in an excavation approximately 92 in. in depth, which was dug up by Employee #2, the backhoe operator. Employee #1 was pronounced dead at the excavation site by the Los Angeles County Fire Department emergency rescue personnel paramedics. Employee #3 and Employee #4 assisted with the rescue of Employee #1 but were not present during the incident of dirt engulfment.
