Event: 09/28/2004
Employee is injured in fall through skylight

At approximately 9:30 a.m. on September 28, 2004, an employee, welder and mechanic, was working within a dual employer arrangement and was at his assigned worksite at the time of the accident. The employee was working on the roof with his supervisor. The employee was cleaning the skylights on the roof of the sorting area in the attempt to clean the roof. The employee had completed the washing of skylights with aid of a wet rag on the area over the sorting areas. The employee proceeded to another portion of the roof to continue cleaning the roof skylights. The employee used a wet rag to rub the surface of the skylight in the same manner as he had for the previous sets of skylights. The skylight broke, and the employee fell to the trash pile below. Interviews were conducted of the employee and his supervisor. The investigation concluded that the accident resulted from approach to a skylight within six feet without fall protection from falls. The employee was transported to Providence Holy Cross Hospital, where he was hospitalized and treated for 7 days.
