Investigation Summary
On March 2, 2004, Employee #1 was driving a flat bed trailer rig to a site to pick up some steel bars. While awaiting for Employee #2, a forklift operator, to place the material on the truck, Employee #1 was asked by Employee #2 to assist him in the placement of a 2-by-4 to support the lifted material. Employee #1 lifted the material with the forks at its end instead of the middle. While the material was being lifted and as Employee #1 was placing the 2-by-4, the beams rolled off the rack and hit Employee #1 on his leg. He was hospitalized with a fractured leg.
