Investigation Summary
At approximately 8:30 a.m. on February 28, 2003, Employee #1, a sauce maker working for a manufacturer of frozen handheld foods, was installing a breather cap on a sauce kettle when his left arm was caught by the unguarded rotating shaft of the agitator that had exposed bolts protruding radially out. The employee's left arm was amputated and he was hospitalized.
