Employee Slips on Foot Tool and Falls

On September 1, 2005, Employee #1 was assigned to fill the Chocolate tanks. To fill the tanks the Employee must use a 4 ft long handle to pull the auger pipe over to fill the tanks. Employee #1 was using the tool and could not grip the auger pipe. When Employee #1 finally grabbed the auger pipe, she pulled on it very hard, and the auger pipe swung over and hit her. Employee #1 fell to the floor and fractured her lower lumbar vertebra. Employee #1 was hospitalized and treated for her injuries.
