Employee Is Struck by Boom from Crane

On September 14, 2005, Employee #1 was working on top of a 66 in. by 20 ft form. Employee #2 operating a lattice boom rail crane was delivering concrete to the top of the form and Employee #1 on top of the form would shovel the concrete down into the sides. While delivering the concrete, the lattice collapsed over the top of the forms. Employee #1 on top the form dove down inside the form but was struck by the lattice. The employee sustained serious injuries and was hospitalized.
