Employee's Leg Is Pinned and Crushed by Forklift Tine

A crew of employees was putting tines on forklift. Employee #1 and a coworker were holding the tines and guiding them onto the forklift when another coworker, driving the forklift, drove forward. The forklift jumped and a tine struck Employee #1's left leg. Employee #1 sustained a broken left leg and was hospitalized. The operator of forklift was untrained.
