Employee's fingers amputated by saw blade

On July 17, 2007, Employee #1 was working in the research and development department, designing a wood piece needed for a new furniture design. The design called for a mold cut in a piece of maple wood. The employee lifted the guard on the Powermatic Table Saw and placed the piece up against the blade. The employee used the push stick in his right hand but used his bare left hand to apply pressure to the left side of the maple. As the piece of maple went over the saw blade, the wood jerked to the right. The employee's left hand crossed over the top of the saw blade. Employee #1 sustained slight finger tip amputations to his first, middle, and ring fingers but was not hospitalized. The employee admitted having had Lockout Blockout training and knowing the rules that prohibited the use of equipment without having the guards in place.
