Investigation Summary

Investigation Nr: 201103793
Event: 05/14/2003
Employee injured when arm burned by welding machine

On May 14, 2003, Employee #1, a supervisor, was requested to repair a welding machine where the ball bearings had fallen out. Once Employee #1 had repaired the welder, he ran it through it's cycle to ensure proper performance. While it was cycling, Employee #1 noticed a sensor wire that was being melted by the heat plate when in the down position. The heat plate operates in a vertical position. When the melted wire was noticed, the heat plate was raised above the table as part of its function. Employee #1 reached under the table and under the heat plate to readjust the wire. When the heat plate finished it's above the table function, it lowered to the under the table position and onto Employee #1's left arm. The corner of the heat plate burned Employee #1's arm down to the bone, and he was hospitalized.
