Employee run over by a forklift his was riding on

Employee #1, a field man, helped a forklift operator unload a flatbed truck of used fencing material. They returned the forklift to the shed. Employee #1 stood on the forks, a foot on each blade. When the forklift drove through a pot hole, Employee #1 lost his balance and attempted to jump clear. The right front tire ran over his leg and he was hospitalized with a fractured ankle and a laceration to the back of his knee.
