Forklift Operator Killed in Forklift Accident

On January 30, 2001, Employee #1 was driving a forklift up a steep grade with a pallet of cinder block and then drove the forklift down the drive way when he hit a curb and ran into tree. Employee #1 was not a wearing seat belt and was ejected from the forklift. Employee #1 was killed.
