Employee injures ankle while ascending a ladder

On January 19, 1999, Employee #1 was using an 8-ft extension ladder to access a five-foot-high garbage dumpster. As she stepped on the rung of the ladder, her foot slipped out and she fractured her ankle. She was transported to a local hospital and admitted into orthopedic surgery for reconstructive surgery of her broken ankle.
