Investigation Summary
At approximately 10:30 a.m. on December 15, 1996, Employee #1 was repairing the roof of a greenhouse after a major wind storm. He was nailing the fiberglass roof together when there was a sudden gust of wind. A portion of the roof was blown off, striking Employee #1 and knocking him down. He fell approximately 7 ft 6 in. through the roof onto the ground. Employee #1 sustained injuries that required hospitalization.
