Employee injured in same-level fall while cleaning floor

At approximately 7:00 p.m. on August 10, 1996, Employee #1, age 18, was cleaning up the floor of the restroom where a toilet had overflowed. While doing so, he slipped and fell onto the floor. Employee #1 sustained a fractured skull, hearing loss, and possible vision loss. The employer was issued citations for failure to notify the division, failure to have an IIPP available during the time of the investigation, and failure to maintain floors in good condition and free of water.
