Investigation Summary
At approximately 7:30 a.m. on June 26, 2002, Employee #1 was installing a metal ridge cap when he stepped back and fell through a 17 ft by 10 in translucent skylight cover and landed on top of the metal trough that was directly under his work location. Employee #1 was transported to hospital where died from severe head and back injuries.
