Employee Amputates Finger Using Unguarded Table Saw

At approximately 3:30 p.m. on September 13, 2001, Employee #1, a new machine operator for a lumber and general hardware retailer, had the index and middle fingers amputated on his left hand as a result of "ripping" wood on an unguarded Delta 10-inch Tilting Arbor Saw. The Serial Number was 90L04594. Employee #1 was instructed to use the 10-inch Delta Tilting Arbor Saw to cut a piece of plywood. Employee #1 set the width, turned on the saw and began to cut the plywood. While cutting the wood, Employee #1 severed his index and middle finger on his right hand. Employee # 1 was transported to Saint Agnes Medical Center for treatment and reattachment of his fingers. No witnesses observed the accident. His employer failed to ensure the saw had the proper guard.
