Investigation Summary
At 10:00 a.m. on April 4, 2001, Employee #1 of an orange packing facility was seriously injured. Employee #1 had worked for the company for approximately ten years as a seasonal packer. On the day of the accident, Employee #1 was placing stickers on packing boxes. The packing boxes were located on a pallet next to the material transfer station. Packing boxes are stored downstairs in a basement type area, formerly used as a fumigation chamber. A hole had been cut out in the floor so that these boxes could be elevated by a fork lift from the basement to the main packing floor. The hole is 6-feet by 8-feet and it is 10-feet to the basement floor. The hole is guarded by railings on three sides and a gate in the front. The fork lift driver elevated the load from the basement, which pushed the gates open. A pallet was placed near the edge of the hole. Employee #1 went to the rear of the pallet to retrieve a carton of boxes and fell through the hole. Employee #1 initially sustained head trauma and two broken vertebrae. She was hospitalized and passed away on May 15, 2001.
