Investigation Summary

Investigation Nr: 201082468
Event: 07/22/2000
Employee's fingertips amputated on table saw

Employee #1, an inmate at a state prison, was assigned to assemble credenzas, desks, and bookcases in the facility's furniture factory. At 10:25 a.m. on July 22, 2000, he left his job area at the front of the building and went to the very back of the building, where the table saws were located. Employee #1 started to use a Northfield table saw to trim the thickness on a 1 1/4 in. tall by 8 in. wide by 15 in. long piece of wood. He stood the board up and was pushing it down on the table saw blade, which was adjusted to a height of 4 in. Employee #1's his hand slipped and struck the rotating blade, amputating the ends of his middle and index fingers. The automatic feed guard had been removed from the saw for servicing. Employee #1 was not authorized to use this saw and was not authorized to be in this area.
