Investigation Summary
At 10:15 a.m. on July 24, 1999, Employee #1 was greasing an unprotected chain and sprocket drive while the machine was running. The grease gun slipped out of his hand and his hand came into contact with the chain and sprocket. The right index finger of Employee #1 was amputated by the chain and sprocket. Due to crushing and mangling his third and fourth digits were amputated just above the knuckles by the surgeon.
