Investigation Summary
On June 25, 2003, Employee #1 was operating a Yale forklift, Model ESC025G4S03, Serial Number N3485-71 at a warehouse. While backing up a hill, the forklift lost power and started to roll down the hill. Employee #1 applied the brakes, but the forklift would not stop. As the forklift continued to roll, it hit a curb and threw him to the ground. Employee #1 attempted to stop the forklift, for a second time, by jumping onto it, but was not successful. The unit struck a parked vehicle, pinning him between the forklift and the parked car. Employee #1 was hospitalized and treated for numerous fractures.
