Investigation Summary

Investigation Nr: 300792900
Event: 01/30/2003
Employee injured when struck by air duct

At approximately 2:55 p.m. on January 30 2003, Employee # 1, of Fraser Boiler Services, was standing approximately 12 to 15 ft away from ducting while a coworker was hydrostatically testing an air duct for leaks using water and compressed air. The coworker completed the test and was using the compressed air to assist in the removal of the water from the ducting. The air pressure going into the ducting was at too great a volume and pressure, and could not be released prior to the duct being over pressurized. This resulted in the failure of the air duct. A section of this ducting separated from the unit and struck Employee #1, fracturing both of his legs. Employee #1 was hospitalized and treated for fractures.
