Investigation Summary
At 3:15 p.m. on December 30, 2002, Employee #1 suffered an industrial accident. Employee #1 was utilizing a circular saw to trim the under side of hotel doors. The employer is in the hotel management business. Employee #1 was the person operating this saw and is a regular employee of this employer, not a contract employee. Employee #1 was working in the maintenance shop at the hotel at the time of this accident. Equipment involved was a circular panel saw manufactured by Safety Speed Cut. The saw was a Milwaukee 2.5 unit, Catalog Number 6410, Serial Number 771B400440092, a 120 Volt, 13 Amp saw unit. Employee #1 was utilizing the circular saw as a panel saw. Employee #1 had removed the guard that covered the blade. While operating the saw, Employee #1's left wrist came into contact with the unguarded blade. This resulted in the amputation of Employee #1's left wrist, and Employee #1 was hospitalized.
