Event: 01/13/2003
Employee injured while unloading boxes from truck

At approximately 2:00 p.m. on January 13, 2003, Employee #1, a shuttler employed by LBC Mabauhay Corporation, was delivering boxes to the company's San Diego office. He was unloading pallets of empty cardboard shipment boxes that were strapped together by plastic bands. Each pallet contained 80 boxes and weighed approximately 500 pounds. The employee pulled on the strap of one of the pallets to move it to the elevator lift gate of the truck when the strap broke causing him to lose his balance and fall. The employee fell approximately 3 feet and hit the metal truck lift with his chin causing a fracture and laceration. He then fell to the paved parking lot and suffered a fractured left wrist. The employee was hospitalized with his injuries.
