Investigation Summary
On November 14, 2002, Employee #1, a maintenance superintendent, was performing a site inspection. He was leaning against a non-operating ice machine supported at the base by 2-in. steel pipes. He started to walk and tripped over the end of a pipe which stuck out approximately 1 ft from the base of the ice machine. He fell on the asphalt parking lot shattering his right elbow and was hospitalized.
