Employee sustains rib injury after falling from ladder

At approximately 12:00 p.m. on November 07, 2002, Employee #1, a maintenance worker, was working for Frog's Club One Inc., in San Diego, California. Employee #1 was assisting a coworker installing fans in the Spin Room of the Frog's Club location. They had completed installation of the fans and Employee #1 was adjusting a fan that was located over a sliding window. The window was open on one side and covered by a screen. Employee #1 was on a 6 ft fiberglass step ladder with aluminum steps, positioned in front of the window. When the employee finished the fan adjustment, he started to descend the ladder. As Employee #1 descended the ladder he lost his footing, and in an effort to catch himself, he reached out in the direction of the window. He was unable to gain his balance due to no firm hand hold and he fell out the window 15 ft to a cement sidewalk. The accident happened on the second floor of the building in the Spin Room (Stationary bikes located in this room). Employee #1 sustained a fractured right rib and right clavicle. He was hospitalized.
