Investigation Summary
At approximately 2:00 p.m. on June 18, 2002, an employee was working on the deck panel of a six-story parking garage construction project. The employee was attempting to remove debris from the concrete form. When he stepped on the platform with his left foot, the form gave way and he fell approximately 30 ft to the dirt. The panel was 16 ft 3 in. by 8 ft long and weighed over 900 lbs. The employee was hospitalized with a broken neck and spinal cord injuries.
