Investigation Summary
At approximately 2:25 p.m. on April 19, 2002, Employee #1 with Entek Manufacturing Inc. in San Marcos California, was in an area adjacent to an operator who was moving a 9,000-lb bucket with an industrial forklift (Caterpillar Model Number T-150, Serial Number 5MB01386, 15,000-lb capacity). The operator lost control of his load and the bucket fell, striking a large steel table. The table then struck Employee #1 and knocked him to the floor. Employee #1 was hospitalized with compound, bi-lateral fractures to both femurs.
