Investigation Summary
At approximately 10:30 p.m. on April 3, 2002, Employee #1 was installing a bolt through a wooden shipping Skid to an enclosure in the northwest end of the building in an area called the General Assembly Area. While lowering the enclosure onto the shipping skid Employee #1 was aligning the tie-down bolt and giving instructions to the crane operator to lower the enclosure. The bolt would not line up with the hole so Employee #1 placed his hand under the suspended enclosure and above the wooden skid in order to adjust the bolt. The enclosure continued to be lowered creating a pinch point and catching and severing Employee #1's fingertip. Employee #1 suffered a partial amputation to his middle finger of his left hand. Employee #1 was hospitalized.
