Investigation Summary
At approximately 9:30 a.m. on January 25, 2001, Employee #1 of Casper Company was operating a mini excavator (Bobcat Model Number 331). Employees were in the process of demolishing concrete prior to installing a cell phone tower. The track on the excavator came loose from the drive mechanism and the employees were attempting to repair it in the field. The excavator tipped over crushing Employee #1's foot when part of the rollover cage landed on it. Employee #1 sustained severe injuries to his left foot which subsequently required amputation. Employee #1 was hospitalized for his injuries.
