Investigation Summary

At approximately 11:00 a.m. on November 13, 2000, Employee #1, an employee of American Tool and Engineering, suffered lacerations to both thumbs while operating a power press brake. The incident was first reported on November 13 at approximately 5:30 p.m. by an employee of Employment Systems Incorporated (ESI). ESI is a contract employment agency, and the primary employer of EEI. American Tool is the controlling employer. The press brake had no effective point-of-operation guarding. Employee #1 was feeding the parts in by hand and controlling the ram by use of a foot pedal. While holding a piece of sheet metal, Employee #1's hand slipped at the point of operation. His thumbs were crushed and had lacerations. Employee #1 went to a local hospital for treatment.
