Investigation Summary
At approximately 11:30 a.m. on December 12, 2000, Employee #1 was operating a gasoline powered wood chipper. He was feeding tree limbs into the chipper. He placed his foot onto the hopper and used his leg to push branches into the chipper blade. His left leg was pulled into the machine and severally damaged up to his buttocks. He was hospitalized and his leg was amputated.
