Employee Finger Is Amputated in a Wire Reshaping Machine

On December 3, 2003, Employee #1 was a regular employee of a manufacturer of pipes for the petroleum industry. Employee #1 was regularly assigned to operate the inverted bull block wire shaping machine. The inverted bull block wire shaping machine had just been added to the production line and its function was to reshape round wire into triangular shape. Employee #1 was squatting down tightening a screw to ease a leak on the lubrication system when he lost his balance upon standing and fell toward the service pit. Employee #1 grabbed on to the machine frame and his right hand was caught and pulled into the bull block by the moving wire. Employee #1's right hand was pinned on the bull block by the moving wire that was applying pressure to the bull block. Employee #1 was released from the pinch point of the machine by his coworkers. Employee #1 was hospitalized and treated for his injuries. The service pit, where the reshaped wire was being collected by the bull block, was located almost directly under the machine and was partially unguarded to protect employees from falling when standing on the platform and performing other tasks. Employee #1, a long time employee at the company who had years of experience operating various type of machines only had 6 months of experience operating the recently purchased machine. The accident was caused by not locking out the power source of the machine. It was determined that the machine was lockable but was not locked out at the time of the accident.
