Investigation Summary
At approximately 12:30 p.m. on September 30, 2003, Employee #1, a truck driver, was driving a propane powered forklift which belonged to the customer to unload a unit of plywood from the delivery truck. Employee #1 offered his help to the customer to unload the plywood sheathings with their forklift since the customer did not have any certified forklift operator available onsite at the time of delivery. After Employee #1 backed the forklift out of the driveway, he began to move forward. Employee #1 stepped on the brake two times but it did not respond and the forklift was gaining speed. Employee #1 decided to side swipe the rear of the delivery truck to stop the forklift. When the forklift hit the rear of the truck, the force of impact displaced Employee #1's body laterally and ejected his hand out which got caught between the corner of the plywood and the rear cage post of the forklift causing his left forearm to break.
